Mission

The mission of Project ECHO® is to develop the capacity to safely and effectively treat chronic, common, and complex diseases in rural and underserved areas, and to monitor outcomes of this treatment.

Project ECHO® is funded in part by a grant from the Robert Wood Johnson Foundation and has received support from the New Mexico Legislature, the University of New Mexico, and the New Mexico Department of Public Health.

Complex Care Clinic

More information

Complex Care Clinic will be scheduled as needed to accommodate any patient cases that are received. Please submit cases by Wednesday of the week prior to Monday's clinic so that we can schedule specialists accordingly.

The focus for this new clinic is on case presentations. Cases will follow a standard form and must be submitted to our team for review no later than the Wednesday before clinic.

Providing primary care for patients with socio-economic, behavioral and medical needs requires a holistic team approach. Teams in the new Complex Care Clinic work together with primary care in an effort to support comprehensive care management for those complex cases. Stay tuned for the new and comprehensive curriculum that will include motivational interviewing, care management, cultural diversity, navigating systems, homelessness, and medical updates.

Consider presenting your cases. Please complete and send the New Case Presentation Form by Wednesday for presentation at Monday’s ECHO Complex Care Clinic session. This clinic starts at noon.

We look forward to working with you through the Project ECHO CCC to improve coordination of care provided to complex patients across the state of New Mexico.
